Students on Post-Completion OPT, STEM, AT

Below is a list of forms for Students on OPT, STEM Extension, and Academic Training (AT) that can be completed online.

Note: If you have already started a specific online form, do not start a new one. Please go to your User Home Page to continue working on your online form.
 
  • Cap Gap Extension I-20 Request - to request a new I-20 reflecting employment authorization extension if your employer has a pending H-1B petition to USCIS on your behalf
  • Change of Education Level - Current F-1 students at the UW at a different level or on OPT recommended by ISS, and you receive admission to a new degree program at UW-Madison to resume studies, must complete this form.
  • ​Copies of Previous Documents - Submit this form if you would like ISS to find a previously issued document in our records. You can also use this form if you need ISS to attest to your immigration record for the duration of your SEVIS ID at UW-Madison.
  • ​​I-20 Reprint Request - If your current I-20 was lost or stolen, or if your current travel signature lines are full or your travel signature will be outdated by the time you intend to re-enter the U.S.
  • Optional Practical Training (OPT) - F-1 students requesting work authorization for up to one year, related to their program of study, must complete this online application with required OPT workshop. Note: should be submitted at least 3 months prior to program completion.
    • USCIS OPT Application Review - Please submit this form to have an ISS advisor review the documents that you will send to USCIS for OPT. No appointment is necessary.
    • OPT Information Update - Please submit this form to report changes during your OPT. You must report changes within 10 days.
  • F-1 Transfer Out - If you plan to transfer to another U.S. school, ISS must first review your SEVIS record before granting access to the new school.
  • ​STEM OPT Extension - F-1 students on OPT after earning a bachelor, master, or doctoral degree in Science, Technology, Engineering, or Mathematics (government approved STEM fields) can apply for 24 months of additional OPT if they have a job or job offer from an E-Verify employer and meet all other required criteria.
    • Report STEM OPT Participation - While on the STEM OPT extension, you must submit a "Validation Report" every 6 months starting from the date the OPT extension begins and ending when your F-1 status or the extension ends, whichever is first.

  • Academic Training (Link to Website) provides J-1 students with practical experience that compliments their program of study and must be directly related to the J-1 student’s field of study, or major 
    • Pre-Completion Academic Training - J-1 students requesting off-campus work authorization during their program for employment training or practical experience
    • Post-Completion Academic Training - J-1 students requesting off-campus work authorization after their program for employment training or practical experience
    • STEM Extension Academic Training - J-1 student degree-seeking Undergraduates and Masters with a STEM eligible major are eligible for an extension on their Post-Completion Academic Training if you will use over 18-months. J-1 students requesting advance off-campus work authorization for employment training or practical experience, related to their field of study. Note: This is only for J-1 students who are currently on Post-Completion Academic Training and in a STEM program.
